If you have Monitoring Service already activated, you can find all info for permitting at your location from your online account here:

Once logged in, navigate to Menu > Monitoring.
Scroll down to the bottom right of this page, and look for Alarm Permit Requirements.
Click the link, and the popup will include all of the info that you need.
